Case management services are provided for people living with HIV and AIDS in our coverage area. Referrals are provided for clients outside of our service area.
Case Management Services
Case management services can help clients obtain medication, find a dentist and pay for dental work, help clients navigate the local healthcare and social service systems, provide emergency money to help pay for utilities, rent, and prescriptions, as well as find and pay for health insurance coverage and certain medical bills. If you feel that you may benefit from this service, call our case managers at our Berwick office, (570) 752-5655, or our Hazleton clinic, (570) 472-2700, to schedule an intake appointment.
Clients enrolled in our agency’s case management program have the ability to choose to take part in our mail-order pharmacy program. This program enables clients to receive their monthly medications directly at their door and provides them with free, non-prescription items monthly, including vitamins, personal hygiene products, nutritional supplements, and many other options.
Free Link-to-Care Client Services
Link-to-medical-care service is provided to those who are newly diagnosed with HIV and/or Hepatitis C or have disconnected from HIV care for some reason. Barriers to client care are addressed in one-on-one sessions with one of our Community Health Workers. If you or someone you know is in need of this service, please click here for our contact information.
